Patient groups in which the use of the Friedewald formula for estimating LDL cholesterol is problematic:

1. Patients who have difficulty complying with the fasting requirement

♦ Geriatric patients

♦ Patients taking multiple medications

♦ Diabetic patients

♦ Pediatric patients

2. Patients with high triglyceride levels

♦ Diabetic patients

3. Patients with type III hyperlipoproteinemia. *

* TYPE III Hyperlipoproteinemia is a disorder where both elevated cholesterol and elevated triglycerides are present in the same patient. It is an inherited disorder (gene defect). This condition increases the risk of heart disease by accelerating the effects of atherosclerosis.
